About 5 million people have dropped their Affordable Care Act marketplace coverage as enhanced federal premium subsidies lapsed and out-of-pocket premiums climbed, NPR reported .
The enhanced subsidies, which had capped what enrollees paid as a share of income and drove ACA sign-ups to record levels, were allowed to expire, so the full premium increase now lands on households directly. The mechanism is straightforward: when the net price rises, the healthiest and lowest-cost customers tend to leave first, which can push premiums higher still for those who remain and concentrate risk in the marketplaces.
